Ilia Topuria has officially received medical clearance to return to competition following the most damaging fight of his professional career.
The former UFC featherweight and lightweight champion revealed on September 16 that he had completed his final medical tests and was cleared to resume his career. The announcement comes three months after Justin Gaethje handed Topuria the first professional defeat of his career at UFC Freedom 250.
Topuria’s return provides an important update after months of uncertainty surrounding his health and future. However, being medically cleared does not necessarily mean he will compete immediately.
Topuria Confirms He Is Back
Topuria announced his medical clearance on social media with a direct message to those who had questioned whether he would return following the loss.
The former champion also made it clear that he has no intention of allowing the defeat to define the next stage of his career. Topuria suffered significant facial injuries during his fight with Gaethje, including fractures to both orbital bones. He did not require surgery, but the damage resulted in a lengthy medical suspension and a mandatory period away from competition.
The California State Athletic Commission initially issued Topuria a 180 day medical suspension unless he received clearance from an oral and maxillofacial surgeon. He also received a mandatory 60 day suspension because of the stoppage loss. His latest medical update indicates that he has now satisfied the necessary requirements.
The Brutal Loss to Justin Gaethje
Topuria entered UFC Freedom 250 as the undefeated former lightweight champion, but his night ended in very different circumstances. Gaethje retained control for large portions of the contest and repeatedly landed heavy strikes. Topuria absorbed considerable damage to his face, with both orbital bones eventually fractured.
After four rounds, Topuria’s corner decided not to allow him to continue. The stoppage handed Gaethje a victory and gave Topuria the first defeat of his professional career.
Topuria had previously won all 17 of his professional fights, including victories over Alexander Volkanovski and Max Holloway during his rise through the UFC. He entered the Gaethje fight as one of the promotion’s most prominent stars. The loss also cost him the UFC lightweight championship.
Topuria Makes Serious Comment About Gaethje’s Gloves
Topuria’s return announcement did not end with his medical update. The former champion also made an accusation directed at Gaethje, suggesting that something unusual may have been done to the lightweight champion’s gloves before their fight.
Topuria said he would make sure Gaethje’s gloves were checked more closely if they meet again. There is currently no evidence establishing that Gaethje’s gloves were illegally altered. Topuria has not provided evidence supporting the allegation, making it important to treat the claim as his accusation rather than an established fact.
Topuria later directed another message toward Gaethje and his manager, Ali Abdelaziz. The comments have added another layer to the possibility of a rematch, although there is currently no confirmed booking between the two fighters.
When Will Ilia Topuria Fight Again?
Although Topuria has been medically cleared, his team does not appear interested in rushing his comeback. According to a report citing Topuria’s physical trainer Jesús Gallo, the former champion has gradually returned to training in Miami. Gallo reportedly indicated that Topuria would be ready to fight from February 2027.
That timeline would give Topuria roughly eight months between his loss to Gaethje and a potential return to competition. The extended recovery period would also allow him to rebuild his conditioning after one of the most physically demanding fights of his career.
Gaethje Rematch or Paddy Pimblett?
The UFC already has several possible directions for Topuria. UFC CEO Dana White said earlier this month that Topuria is ready to fight and confirmed that the promotion remains in contact with his team. White also identified both a rematch with Gaethje and a fight against Paddy Pimblett as attractive possibilities.
A rematch with Gaethje would immediately revisit the result that changed Topuria’s career for the first time.
The Pimblett matchup has a different history. Topuria and Pimblett have exchanged hostile comments for years, creating a rivalry that has existed well before a fight between them became realistic. For now, neither matchup has been officially announced.
Topuria’s Next Chapter
Topuria’s medical clearance removes one of the biggest obstacles standing between him and a UFC return. The former champion now has time to recover fully, return to regular training and prepare for another run in the lightweight division. Whether that leads to another meeting with Gaethje or a long awaited showdown with Pimblett remains undecided.
What is clear is that Topuria intends to continue competing. After suffering the first defeat of his career and spending months recovering from serious facial injuries, the 29 year old has now confirmed that his UFC career is moving forward.
